How To Book Your Service

Step 1: Call 773-238-5555

Step 2: You will asked a series of questions to determine if our services are right for you (if ineligible, we will offer a referral)

Step 3: If eligible, you will need to complete several forms before services begin:

  • Letter of Understanding

  • Intake Form

  • Consent of Service

  • Client Rights and Responsibilities

  • Fees of Service

  • HIPPA and Privacy Guidelines

  • Release of Information

  • Demographic Survey (anonymous)

  • Cross Cutting Symptom Measure (overview of current symptoms)

Step 4: Your insurance will be verified, and you will be notified of deductible fees and copays required.

Step 5: We will find a good date and time to do your appointment.

Important: No one is refused service due to inability to pay and there is a discount application for clients who have special circumstances and need support. If you are a self-pay or on deductible, you will have to make a deposit to reserve your appointment. There are fees for no call no show and late cancellation. You may or may not receive a reminder call about your appointment.  You are ultimately responsible for your attendance.
